In the Netherlands, an estimated 2 million people suffer from back pain. The cause and localization of symptoms vary widely. A common localization of back pain is the lower back. This is often described as back pain with radiation to the legs/buttocks. The problem is often located in the Sacro-Illiac joint (SI joint).

What are the sacroiliac joints?

The sacroiliac joint is the joint between the sacrum (Sacrum) and the pelvis (illium). It is a relatively stiff joint with little range of motion. The joint sits at the back of the pelvis and provides a transition from the spine to the pelvis. This causes the sacroiliac joint to transfer the full weight of the torso to the legs.

Because the SI joint has a large and constant pressure, many people develop pain in the joint. Around the joint, tendons, ligaments and muscles work together to strengthen it. These structures can also become overloaded, causing pain.

Pain in the sacroiliac joint

When a person suffers from symptoms in the SI joint, it is often described as “SI syndrome. The main feature of SI syndrome is pain in the lower back, often at the site of the joint. Because many nerves run along the SI joint, pain can sometimes radiate to the buttock, groin or thigh. The pain often increases with prolonged sitting or lying on the joint. Pain may also increase precisely while moving or lifting.

People with long-term pain in the SI joint may experience difficulty with various movements. This can sometimes have the consequence of not being able to climb stairs, in severe cases even not being able to walk at all with the painful side.

The causes of pain in the sacroiliac joint

There are several causes of pain in the SI joint. For example, symptoms may arise when the muscles and ligaments become loose after pregnancy, hip surgery, a fall or other accident.

The pain symptoms can also be caused by asymmetrical movements in the left and right joints. This may be the case, for example, in people with poor gait, or a leg length difference. It is also common in people with a condition that causes damage to the joint. These include osteoarthritis, certain congenital defects and certain autoimmune disorders.

Finally, pain in the SI joint often occurs when the joint is overloaded. This is the case, for example, of people who exercise intensively or do physically demanding work. Pain in the SI joint can also develop in people who have poor sitting posture that they maintain for long periods of time.

Our care for SI joint pain

Complaints in the SI joint can be very painful, painkillers can effectively alleviate this. Unfortunately, painkillers do not address the cause of the pain. To effectively relieve pain in the SI joint, it is important to work on body posture and strengthen the muscles in the SI area. Strengthening the muscles then also ensures that the symptoms are less likely to return.

A chiropractor can help both relieve pain and improve mobility. For this, there are several techniques a chiropractor can use, such as spinal manipulation. The chiropractor moves and rotates the back so that the SI joints stabilize. This often leads to immediate pain relief and contributes to the healing process.
